Senior AI Backend (Node.js) Developer

Format: Remote (B2B contract from EU)

MEDvidi is a mental health technology platform that connects patients with licensed physicians in the United States through online care.

We are growing our Engineering team and opening a role for a Senior Backend Developer with a strong product mindset to join our AI Automation team - an opportunity to shape real AI products used by thousands of people. In this role, you will work at the intersection of backend engineering and applied AI, designing scalable systems and directly seeing your work reach production and create impact.

Responsibilities:

- Design and build backend architecture for AI-powered automation systems with a focus on performance, scalability, and fault-tolerance
- Integrate and scale LLM-based product features, collaborating closely with AI engineers and product teams
- Implement and evolve agent-based systems (ReAct, supervisor-agent models) to support complex decision-making flows
- Optimize Retrieval-Augmented Generation (RAG) pipelines using structured and unstructured data
- Develop and maintain robust APIs for internal services and external integrations
- Ensure strong observability, monitoring, and alerting (Grafana, OpenSearch)
- Participate in code reviews, contribute to architecture discussions, and promote engineering excellence
- Maintain technical documentation for newly designed systems and APIs
- Collaborate cross-functionally to align backend systems with product goals and user needs

Required:

- 5+ years of backend development experience using Node.js
- Solid expertise in building scalable, high-performance backend systems
- Experience integrating LLMs in production - delivering real product features, not only demos or pet projects
- Experience with RAG pipelines (embeddings, vector DBs, top_k retrieval, ranking, context enrichment)
- Familiarity with agent orchestration (LangGraph, AutoGen, SmolAgents or similar)
- Understanding of ReAct paradigm and agent-supervisor role patterns
- Skilled in prompt engineering and quality evaluation (AI Eval frameworks)
- Ability to work with structured LLM outputs (JSON/YAML) and enforce validation
- Experience with PostgreSQL (schema design, optimization)
- Experience with Grafana and OpenSearch or similar observability stack
- Strong API development skills
- Experience with testing practices (unit, integration, CI/CD automation - Jest or similar)
- Fluency in English and Russian
- Experience with Amazon Web Services (AWS)

Nice to Have:

- Experience designing configuration-driven internal tools enabling non-technical teams (PMs, analysts) to manage logic or content
- Experience building voice-based AI or telehealth automation systems
