Они требуют две вещи:

1) счета
2) заполненную форму с датами въездов и выездов за текущий и предыдущий год

Я еще прилагал сканы соответствующих штампов в паспорте, но уже не помню, было ли это формальным требованием.

Нашел, что они мне тогда написали:


Regarding the application with number XXXXXX and in order to check the habitual residence in the Republic in accordance with the provisions of the Article 16 of the GHS, please send us in a scanned and clear PDF format (photos will not be accepted)ALL the following documents:
ALL passport pages with the seals of arrival and departure (from the first to the last page including the blank ones) from the Republic.
The declaration of entry & exit dates to the Republic of Cyprus (attached)
Copy of tax return of the previous year submitted in the Republic of Cyprus (if applicable).
