Title: Individual’s Self Declaration Form regarding the connection and existing infrastructure that exist in the facilities described below.

1. I haven’t benefited in the past from the Measure “Broadband connectivity voucher scheme”.
2. There wasn’t available in the past nor is now a broadband connection of 100Mbps (or above) at the premises where I live and are described above.
3. The pre-existing infrastructure until my premises (including the building infrastructure, the infrastructure up until my premises and my terminal equipment): was not sufficient to provide 1 Gbps download / 150 Mbps upload speed services.
4. I understand that providing incorrect or intentionally misleading information may have legal consequences and may result in my disqualification from the Measure.
5. I authorise the Provider, whose name is mentioned below to collect on my behalf the money of the voucher that I am entitled to, in the context of the Measure “Broadband connectivity voucher scheme”.

I have read and accepted the conditions of participation stated in the “Broadband connectivity voucher scheme” Implementation Guide.
